Welcome to Galloway Academy. In order to enroll your child in our school, you will need to complete the pre-enrollment application. Once your application has been reviewed for acceptance, you will be notified of the decision.
Should your child be accepted for enrollment, you will then be required to submit the following:
- Copy of Immunization/Health record/certificate
- Copy of Certified Birth Certificate
- Copy of the student’s social security card
The following forms must also be completed and submitted after you have received the acceptance notice:
- Transfer Request Form
- Photo/Publicity Release Waiver
- Medical Liability Release Form
- Proof of Insurance Form
